SEN Transition

We recognise that transition times can be difficult for any child, especially those with SEND and take steps to ensure that any transition is as smooth as possible.

How we support children/young people with SEN starting at our school 

When children with SEND start our school we try to put in place an enhanced transition period wherever possible.  The class teacher makes sure that they have spoken to the child and families and where possible met them before they start school whether this is in their current provision, in school or at home.  We will meet with professionals before the child begins schools to ensure that we have the correct provision in place to meet their needs.

How we support children/young people with SEN moving between classes in our school 

Where ever possible we have a good period of transition between moving classes.  All of our teachers work closely together and often already know their next class well.  For children with SEND, additional measure are put in place to make sure they feel comfortable and know the staff they will be with in each class.

How we support children/young people with SEN leaving our school 

To ensure a smooth transition to their next setting when leaving our school we always try to speak to the staff from the next school to pass any relevant information over and to ensure that data and paperwork are passed on securely.  We will prep the child who is leaving about their next setting and try to make sure they are making progress with the preparation for adulthood pathways.
